One of the biggest problems with a massive Northern California Earthquake is the problems with the Hospitals, as very few of them are earthquake retrofitted. So an Earthquake in the Bay Area of a magnitude of 8.0 or higher will mean there will be no Hospitals still standing or without damage to take the estimated 50,000 to 250,000 seriously injured population and subsequent after shocks will make it even worse.
Currently there is a deadline of January 2013 for all the Hospitals to unblemished their retrofit, but the chances of a huge Earthquake hitting before then are extremely high. Still most feel that even the 2013 date will not be met. Many say there is just no possible way. Not to mention the Healthcare business is under extreme costs stress as it is due to over regulation, labor disputes and lawsuits.
